You and XPO share the cost of medical and dental coverage. You pay the total cost for vision coverage. Log onto myXPO Portal, click on Your Benefits, then Benefits for rate information on all of your benefits.

About Cost-Sharing

You and XPO share the cost of medical coverage. For medical coverage, your share of the cost is determined by the coverage level you elect, your employment status (for example, hourly or salary) and business unit. Tobacco Surcharge

If you or your spouse uses tobacco, you are required to pay a tobacco surcharge of $100 per month in addition to the medical plan cost. If you want to quit, XPO provides the Tobacco Free program through Anthem at no cost to you.

If you complete Anthem’s Tobacco Free program, you can ask for the tobacco surcharge to be removed for the remainder of the plan year. Removal of the tobacco surcharge is available to you and your eligible dependents. If you think you may be unable to complete the Tobacco Free program, you may qualify for an opportunity to have the surcharge removed by different means. Contact the XPO Benefit Center and we will work with you (and, if you wish, with your doctor) to find a tobacco cessation program that is right for you in light of your health status.

Spousal Surcharge

If your spouse does not enroll in coverage available through his or her employer, you will be required to pay a monthly spousal surcharge of $151.66. The surcharge applies if your spouse:

  • Is employed
  • Is offered medical coverage through his or her employer
  • Declines medical coverage from his or her employer
  • Enrolls in XPO medical coverage

You will not have to pay the surcharge if your spouse enrolls in medical coverage through his or her employer and is also covered as an eligible dependent under your XPO coverage.
